The risk of vehicle intrusion is well known in railways and there are statutory requirements on managing the safety risk of construction activities in the vicinity of railway. Yet, such kind of tragedies keep repeating - what went wrong? A railway incident does not come from just bad luck. The safety of a railway system is assured by rings of safety barriers to prevent reasonably foreseeable accidents. The Speakers will discuss the important of a robust risk management regime to drive the safety culture in controlling railway safety risks, and address the importance of learning from past incidents, local and overseas. The application is transferable to other industries as well.
